Description: The Adult Self-Report Scale Version 1.1 (ASRS) is a self-report questionnaire designed for adults. This questionnaire aims to characterize the presence of signs and symptoms of ADHD. The ASRS is a 6-item scale. Responses to the six items are Never, Rarely, Sometimes, Often, Very Often. Our team has received permission from the assessment creators to incorporate it into our measurement based care system. This assessment is available in Spanish and English.
Scoring: For the first three items, a value of ‘1’ is assigned to responses of Sometimes, Often, or Very Often; a value of '0’ is assigned to responses of Never or Rarely. For the latter three items, a value of ‘1’ is assigned to responses of Often, or Very Often; a value of '0’ is assigned to responses of Never, Rarely or Sometimes.
The values of ‘1’ and ‘0’ associated with the six items is then summed to create a Total Score. This Total Score has a minimum value of 0 and a maximum value of 6. A total score of 4 or higher indicates that the patient is reporting symptoms which may be consistent with adult ADHD.
