Description: This assessment was created internally at Sheppard Pratt by the Department of Point-of-Care Research & Innovation (PoCRI). We queried Chat GPT to generate a variety of self-report items regarding ADHD. A team of clinical experts reviewed, refined, and revised these generated items to create the present items. Our team of clinical experts, in consultation with each other and with literature review, generated an additional item added to this assessment which may be used as an indicator of possible over-medication.
Scoring: Four items which measure presence of ADHD-related signs and symptoms are included in this scale. Likert-scale style responses, associated with numeric values, are present for these four items: 1, Never or Rarely; 2, Occasionally; 3, Sometimes; 4, Often; 5, Very often or always. The responses to these four items are summed to provide a Raw Score. This Raw Score has a minimum of 0 and a maximum of 20.
The fifth item measures possible indicators of over-medication. This fifth item has the same scale of Likert-style responses (i.e., 1, Never or Rarely; 2, Occasionally; 3, Sometimes; 4, Often; 5, Very often or always). This item is not included in the Raw Score calculation. If the response to this item is greater than 1, text will display on the report which reads: Investigate possible adjustment of stimulant dose or frequency.
