Description: This 8-item short form developed by Swanson, Nolan and Pelham provides a way to track DSM-5 symptoms of ADHD for children and adolescents; specifically, children and adolescents 6-17 years of age. This questionnaire is a proxy-reported measure; that is, the caregiver will complete this questionnaire about the patient. Dr. Swanson, the developer of this assessment, has provided permission to our team to integrate this questionnaire into our measurement based care system. The SNAP-IV is available in Spanish and English.
Scoring: Each of the 8 items’ responses are associated with a numeric value: 0=not at all; 1=just a little; 2=quite a bit; 3=very much. The values of these 8 items are summed; this item summation provides the Total Score. This Total Score has a minimum of 0 and a maximum of 24.
The Total Score is divided by the number of completed items (i.e., 8) and rounded to the nearest whole number to provide the Average Total Score. The Average Total Score is a 4-point scale with a minimum of 0 and a maximum of 4. The Average Total Score has the following ranges: none (0), mild (1), moderate (2), or severe (3).
