Transition to Young Adulthood Assessments (14+)

Description: For patients ages 14+. The Transition to Young Adulthood (TYA) battery of assessments was created internally at Sheppard Pratt by providers in the Neurodevelopmental Disorders Clinic. This assessment is intended to gather information about and identify potentially unmet needs across the domains of Transportation, Living Situation, Healthcare Providers, Employment, Education, Financial Management, and Safety. All assessments are available in self-report versions intended for the patient to complete, as well as proxy-report versions intended for the caregiver to complete (Note: the TYA Safety assessment has a proxy-report version only). 

Scoring: The assessments are intended to be reviewed by the provider as qualitative measures, as opposed to quantitatively scored. In each assessment, one or several items are used to indicate a potential unmet need; the presence of the unmet need is ultimately determined by the provider and the patient during the clinical encounter.  see below

  • TYA Transportation: Do you have a driver’s license?/ Does the patient have a driver’s license?
  • TYA Living Situation: Are you satisfied with your current living situation?/Are you satisfied with the patient’s current living situation?
  • TYA Education: Are you currently in school?/Is the patient currently in school?
  • TYA Employment: Are you satisfied with your current employment status and situation?/Do you feel the patient is satisfied with their current employment status and situation?
  • TYA Healthcare Providers: Do you have a current adult primary care provider? AND-OR Do you feel satisfied with your specialists?/ Does the patient have a current adult primary care provider? AND-OR Does the patient feel satisfied with their specialists?
  • TYA Financial: Do you manage your own money?/Does the patient manage their own money?
  • TYA Safety: Do you have concerns regarding the patient’s safety?
