AUDIT-C (adult/pediatric)

Description: A brief alcohol screening instrument that reliably identifies persons who are hazardous drinkers or have active alcohol use disorders (including alcohol abuse or dependence). If patient has a positive screen, PROMIS Alcohol Use will follow the AUDIT-C.

Adult self-report ages: 18+ years
Pediatric self-report ages: 8-17 years
Pediatric proxy-report (patient) ages: 5-17 years

Self-report measure meets the requirement for MIPS Quality Metric ID #431: Alcohol Use Screen.
