The Columbia-Suicide Severity Rating Scale - Since Last Visit (C-SSRS) is used to assess suicide risk since a patient was last seen by a provider/asked.
The Columbia-Suicide Severity Rating Scale - Past Month (C-SSRS) is a screening measure used to assess suicide risk in the past month. For patients who screen positive on the Past Month version, they will receive The Columbia-Suicide Severity Rating Scale - Intensity of Ideation (C-SSRS).
Scoring for Since Last Visit and Past Month: higher score = higher risk/severity of suicidal ideation
Maximum score of 6.
- 0-2: low risk/severity
- 3: moderate risk/severity
- 4-6: high risk/severity
The Columbia-Suicide Severity Rating Scale - Intensity of Ideation (C-SSRS) is a screening measure used to assess the severity of suicidal ideation endorsed by a patient. Patients who screen positive on the Past Month version will receive this measure.
Self-report ages: 8+ years
Pediatric Proxy-report (patient) ages: 5-17 years
Scoring for Intensity of Ideation: higher score = more intense suicidal ideation
Score ranges from 2 to 25.
- 2-5: low/mild ideation
- 6-10: moderate ideation
- 11-15: moderate severe ideation
- 16-20: severe ideation
- 21-25: very severe ideation
