Description: The NIDA-Modified Substance Use Screener is a self-report measure that assesses substance use in the last 30 days. If substance use is affirmed, an additional item regarding types of substances used will be presented for the patient to select.
PROMIS Substance Use measures the severity of substance use in the past 30 days.
For positive screens, patients will receive the PROMIS Severity of Substance Use (Past 30 Days) and the Substance Use Treatment Interest Screen.
Self-report ages: 8+ years
Pediatric proxy-report (patient) ages: 5-17 years
