Description: Adult version focuses perceptions of sleep quality, sleep depth, and restoration associated with sleep.
Pediatric version assesses reported thoughts of one's sleep quality, and perceived difficulties with falling or staying asleep. Conceptual facets include sleep quality, sleep onset, and sleep continuity.
If a patient scores moderate or higher, they will receive PROMIS Sleep-Related Impairment as a supplemental assessment.
Adult self-report ages: 18+ years
