PROMIS Sleep-Related Impairment (adult/pediatric)

Description: Focuses on perceptions of alertness, sleepiness, and tiredness during usual waking hours, and the perceived functional impairments during wakefulness associated with sleep problems or impaired alertness. 

Patients will receive this measure if they score moderate or higher on PROMIS Sleep Disturbance.

Adult self-report ages: 18+ years
Pediatric self-report ages: 8-17 years
Pediatric proxy-report (patient) ages: 5-17 years
