For admission (or first PROM) and post-discharge, patients will automatically receive the questionnaires listed below. Each questionnaire is available in Spanish and has between 3 and 8 items. Some measures are not available for certain age ranges due to the lack of Spanish translation or the lack of validation for the specific age range.

For follow-up PROM questionnaires, patients will receive PROMIS Depression and PROMIS Anxiety. You can add additional questionnaires with the Select Questionnaires button in the PROM panel
The questionnaires are available for all ages and have Spanish translations unless stated otherwise. Select the domains that the patient will complete and select confirm.
These changes will apply to all future follow-up PROMs unless changes are made by the provider in the Select Questionnaires panel.
Selected questionnaires will appear under Questionnaire Selection once saved.
