What are Proxy Reported Outcome Measures?
In addition to Patient Reported Outcome Measures, in which patients complete a series of self-reported questionnaires, the dashboards also incorporate measures which allow for a caregiver to complete questionnaires about the patient’s functioning. We refer to these measures as Proxy Reported Outcome Measures.

Sending Proxy Reported Outcome Measures
Proxy Reported Outcome Measures are not automatically sent out for all patients.
If you wish to have Proxy Reported Outcome Measures sent to a patient’s caregiver, providers will need to manually select the desired Proxy Reported Outcome Measures and determine the frequency with which they will be sent out. Automatic email and text notifications will be sent to the same email and cell phone number listed in IDX for the patient.
The Proxy Outcome Measures will be sent in an separate email from the Patient Reported Outcome Measures, with clarifying instructions. Please see below for an example of the email notifications, text notifications, and the questionnaire introduction screen.
If I exclude a patient from receiving self-report measures, can Proxy Reported Outcome Measures still be sent to the caregiver?
Yes- even if you exclude a patient from receiving the self-report Patient Reported Outcome Measures, questionnaires can still be sent to the caregiver per the settings entered by the provider in the Proxy Reported Outcome Measures column on the dashboards.
